Coordinating Patient Care

Patient care that is well coordinated means that the medical office gets test results to patients, sees that the doctor is up-to-date about all of the patients’ care and that everyone that the patient saw has the right information about their health problem. Superior medical groups train their staffs and use systems to handle and communicate information properly to make sure that patients don’t fall through the cracks.

These results are based on a survey of a sample of medical group patients.

How well do the medical group’s doctors and staff do in seeing that all of the doctors and staff who care for the patient have the right information all the time?
