Reporting a Question, Concern or Complaint

If you feel an HHSA employee may have broken the law as it relates to health care, social services, privacy or confidentiality, you may report your concern using any of the following options.

  1. Talk to the employee's supervisor or manager
  2. Telephone the Compliance Office at 619-515-4244
  3. Telephone the toll-free Compliance Hotline (24 hours a day / 7 days a week) at (866) 549-0004
  4. E-mail the Compliance Office
  5. Complete the "Compliance Concern" reporting form and mailing it to:

Reports made using any of the above options will be handled confidentially as practical and/or as allowed by law.

You may remain anonymous, but doing so may impact our ability to fully investigate and resolve your complaint.
