Domestic Violence Fatality Review Team

The County of San Diego Domestic Violence Fatality Review Team (DVFRT) is a coordinated effort between the County of San Diego Health and Human Services Agency's Office of Violence Prevention, and the County of San Diego District Attorney's Office. This multi-disciplinary team's purpose is to make recommendations for system's change, in order to prevent future deaths from intimate relationship violence.

DVFRT Mission

The current mission of the DVFRT is "to prevent future deaths from intimate relationship violence (IPV) by utilizing a systematic, confidential, multi-agency death review process, and to identify system's gaps in order to expand effective violence prevention policy and coordinated strategies."


Multidisciplinary Review Team

The County of San Diego Domestic Violence Fatality Review Team (DVFRT) is a multi-disciplinary review team. It was established in October 1996 by the County of San Diego Board of Supervisors. It is a collaborative effort between the HHSA Office of Violence Prevention (OVP) and the County of Diego District Attorney's office, in addition to the Sheriff's Department Domestic Violence (DV) Unit, San Diego Police Department's DV Unit, Probation Department, Pretrial Services, San Diego City Attorney's Office, The Family Justice Center, Medical Examiner's Office, Child Welfare Services (HHSA), Children's Hospital, Family Court Services, Children's Mental Health Services (HHSA), Juvenile Probation Department, District Attorney's Office Victim Assistance Program, Alcohol and Drug Services (HHSA), Public Health Nursing (HHSA), Aging and Independent Services (HHSA), Navy Family Advocacy Center, Naval Medical Center, San Diego Superior Court DV Committee, San Diego Volunteer Lawyer Program, Clinical Forensics Representative, GLBTQ Community Representative, and Youth Violence Prevention Specialist (HHSA).


The DVFRT meets once a month. All meetings are closed sessions and kept strictly confidential. Each member is required to sign a Confidentiality Statement. All records and discussions shared during case review are protected under California Penal Code Sections 11163.3-11163.5. CA Penal Codes 11160-11163 Domestic Violence

Team's Accomplishments:

The team has been recognized by the California State Attorney General's Office as a model for domestic violence fatality review teams. The team has participated on the State of California Domestic Violence Death Review Team Statewide Protocol Advisory Committee, and has presented at various National and International domestic violence conferences. They have also been host to a number of national domestic violence fatality review teams that are just forming. Other best practices include:

  1. Increased team membership by including representatives from research, Epidemiology, the County of San Diego's Public Defenders Office, faith community, Batterer's Treatment, public health nursing, and the development of a Family Involvement Committee for case review purposes.
  2. Development of a Risk Assessment Check List for court Judges, to assist domestic violence victims in obtaining a Restraining Order against a perpetrator.
  3. Development of a state-of-the-art database, for risk factors associated with domestic violence-related fatalities.
