This page provides an overview of AIDS Case Management services offered by the County of San Diego HIV, STD and Hepatitis Branch (HSHB).

AIDS Case Management, also known as ACM, provides assistance to clients who are HIV positive in accessing medical and support services. The program focuses on clients who are currently incarcerated in a San Diego County jail or who require inpatient substance abuse treatment.

ACM works with HIV positive clients in San Diego County jails who are waiting to be released back into the community. While in custody, clients may participate in ACM to develop a post-release plan that addresses access to medical care, housing, and substance abuse treatment.

A component of ACM is the Intensive Case Management program, or ICM, which offers inpatient treatment to HIV-positive individuals requesting treatment for addiction to alcohol and drugs. To be eligible for ICM and placement into a private treatment program, clients must demonstrate a desire to end substance abuse, have an income of less than $1,100 per month, and have an HIV diagnosis. ACM staff will work with each client throughout the treatment process to promote successful outcomes and to assure linkages with primary care and other services as appropriate.

AIDS Case Management and ICM are funded by Ryan White HIV/AIDS Treatment Modernization Act Part A funds. The ICM program is funded by Housing Opportunities for Persons with AIDS (HOPWA) through a contract with the County of San Diego Department of Housing and Community Development.
