Personal Care Services Workgroup
Goal: To forward a recommendation to the Planning Committee on how to best integrate personal care services into the fully integrated model (HSD+) in a manner that is consistent with the LTCIP stakeholder vision.

Provider Network Development, Education and Member Services Expansion Workgroup
Goal: Recommend minimum performance standards and requirements for health and social service provider network make-up, development, required training and education and member services expansion for Acute and Long Term Care Integration (ALCTI)/Healthy San Diego Plus (HSD+) that address the unique needs of seniors and persons with disabilities.

Care Management Workgroup
Goal: Recommend care management model, tools, and process with minimum performance standards and requirements for Acute and Long Term Care Integration (ALTCI)/Healthy San Diego Plus (HSD+) that address the unique needs of seniors and persons with disabilities.

Community and Cultural Responsiveness Workgroup
Goal: Recommend community and cultural minimum performance standards and requirements for Acute and Long Term Care Integration (ALTCI)/Healthy San Diego Plus (HSD+) that address the unique needs of seniors and persons with disabilities.

Advisory Workgroup
Goal: Make final decisions and recommendations for inclusion in the plan.

Community Network Development Workgroup
Goal: To provide community-based health and social services organizations with opportunities for learning and relationship building with respect to developing and implementing community-based, integrated systems of Long Term Care.

Finance/Data Workgroup
Goal: Determine the financial feasibility of the proposed LTCIP for San Diego County.

Health Plan Workgroup
Goal: Explore use of Healthy San Diego model for potential service delivery system for LTCIP.

Options Workgroup
Goal: Explore and develop a recommendation for a fee-for-service option and an '"other" option, in addition to the fully integrated model (Healthy San Diego Plus), as directed by the Board of Supervisors.

Since July 2002, the Options Workgroup has evolved into a multi-faceted approach to long term care integration that involves the following three complementary strategies. Click here for the highest-level overview of the three strategies.

Network of Care/Aging and Disability Resource Center (single point of information and access)
Goals: (1) develop a warehouse of information on self-care management, healthy lifestyle choices, and other information to improve quality of life for our aging and disabled populations; (2) improve the user-friendly nature of information that links health and social services for these user groups; (3) enhance the use of the system as a communication tool, including for the two LTCIP service delivery strategies (HSD+ & Physician Strategy); and (4) perform formalized testing to assess the adequacy of the database and function of this information for physicians, community-based providers, consumers, caregivers and Call Center staff.

Physician Strategy (managed fee-for-service delivery model)
Goal: Similar to that of Healthy San Diego Plus (HSD+): quality, consumer-centered health and social services through improved chronic care management, but without the capitation risk.

Healthy San Diego Plus (voluntary, acute and long term care integration (ALTCI) service delivery model)
Goal: Continue efforts toward HSD+ development, a voluntary, fully integrated service delivery model, with a capitated payment from Medi-Cal, and from Medicare for the "dually eligible" that blends funding, reduces gaps, and removes barriers to healthcare access.

Mental Health & Substance Abuse Workgroup
Goal: Make a recommendation to the Planning Committee regarding the inclusion of mental health and substance abuse services, programs and populations in LTCIP.

Stakeholder Group for Persons with Developmental Disabilities
Goal: Make a recommendation to the Planning Committee regarding the inclusion of people with Developmental Disabilities in the LTCIP.
