TEAM SAN DIEGO, an eight-module online program, emphasizes team-based care in a healthcare system that is increasingly fragmented and needs improvements in chronic care management.

This program includes a companion half-day in-class training after taking the online course. The training is designed to create "virtual teams" to improve care coordination among all health and social service providers to help patients effectively participate in managing their own health and health care outcomes. TEAM SAN DIEGO is supported by a grant from the California Endowment and Alliance Healthcare Foundation. It is a joint initiative of George Mason University, UC San Diego Extension, and the County of San Diego's Long Term Care Integration Project. Continuing Medical Education (CME) for physicians is sponsored by the University of California, San Diego School of Medicine.
