Broad and active participation of key stakeholders including providers, consumers and consumer advocates involved with the care of frail elderly and disabled in San Diego is pivotal to the success of the LTCIP planning process. To date, local stakeholders have devoted more than 10 years and 30,000 hours of time and in-kind support to the LTCIP. All LTCIP planning meetings and workgroups are open to the public, and we welcome new members.

The next LTCIP Planning Committee Meeting will be held on a TBD in February. Please monitor this announcement page for information about upcoming meeting dates and topics.

Aging and Disability Resource Connection (ADRC) Update

San Diego County’s ADRC is a highly visible and trusted place where people of all incomes and ages can turn for information on the full range of long term support options and a single point of entry for access to public long term support programs.

The ADRC has received two grants to support the Network of Care Strategy under the LTCIP.

The Medicare Improvement for Patients and Providers Act (MIPPA) grant will fund Medicare beneficiary outreach and assistance to improve coordination and enrollment of beneficiaries in the Medicare Savings Plan (MSP) or the Low-income Subsidy (LIS) to help pay for the Medicare Prescription Drug Benefit (Part D) premiums.

The ADRC Enhancement Grant will fund enrichment activities to it’s core functions including:

  • Information & Assistance
  • Short-Term Care Coordination
  • Long-Term Care Options Counseling
  • Care Transitions

Updates on the LTCIP’s other two strategies will be provided at February’s LTCIP Planning Committee Meeting.
